Position Summary:
Serves as a member of the health care team, assisting individuals/families and the treatment team through the provision of psychosocial assessments; advocacy; education; treatment and discharge planning for all patients admitted to the inpatient unit in observation, acute medicine and swing bed status.. Social Workers will screen all patients to determine extent of social work services needed by each patient.

Accountabilities/Essential Job Functions:
  1. Provides a psychosocial assessment of patient, family or appropriate support systems in order to implement crisis or support interventions and to maximize coordination of services.
  2. Initiates discharge planning with patients and their families to develop a plan that meets the patient’s needs in both short and long term planning. Examples: coordinates residential placement, transportation, home health, homecare, mental health follow-up.
  3. Provides counseling/education for patient, family, support systems regarding adjustment to illness and hospitalization, end of life issues, decision-making re: substance abuse, domestic violence, elder abuse, and child abuse.
  4. Resolution or identification of other problems related to the patient’s transition within the health care setting.
  5. Coordinates, negotiates and advocates for patient’s needs with payors and external case managers.
  6. Maintains verbal and written communication with the multidisciplinary team on patient’s psychosocial adjustment, interventions and goals through medical record documentation and consultation.
  7. Participates in social work and hospital-wide quality assurance activities.
  8. Participates in hospital interdisciplinary activities, such as committees, team planning meetings, informational meetings, patient/family conferences.
  9. Actively participates in own performance review, being open to constructive feedback and to integrating recommendations into practice.
  10. Maintains current professional license, including requirements for ongoing continuing education.
  11. Attends community social work, interdisciplinary health care and community service agency meetings in order to represent Snoqualmie Valley Hospital in planning for services, resources. Educates community about Snoqualmie Valley Hospital programs and services and education.

Qualifications

Education/Training: Masters Degree in Social Work

Experience: Supervised experience as a social worker in a health care setting.

License/Certification: Registration as a Social Work Associate Advanced (SWAA) or Social Work Associate Independent Clinical (SWAIC). Licensed as a Social Worker in Washington State (LICSW, LASW) preferred.

Environmental and Job Hazards: Potential exposure to patient infectious disease and body fluids. Function in emergencies and incidents involving patient care

Physical and Mental Demands: Assist with moving and transferring patients (involves pushing and pulling). Natural or corrected near-visual acuity is necessary to read patient charts, physician orders, computer screens. Natural or corrected hearing is necessary to communicate effectively with others.

Required Skills
