Interviews patients to collect required registration data, and enters patient information into computer database. Obtains required signatures, assembles patient files, and forwards to nursing units. Directs or escorts patients to appropriate departments. Performs related duties such as filing reports, assembling pre-admission forms, answering telephones, and updating computer records.

Accountabilities/Essential Job Functions:

    • Interviews patients or their representatives to collect and verify required data for registration form which includes name, address, phone number, attending physician and individual or agency responsible for hospital bill.
    • Enters patient information into computer database, as well as other information such as tests ordered by physician, and direct from lab slips.
    • Obtains required signatures on forms according to established procedures, and ensures information recorded is complete and accurate.
    • Assembles patient files which include registration forms, test requisitions, and photocopies of insurance cards, driver's license, and pre-admission orders. Delivers patient information and forms to appropriate units immediately.
    • Answers telephones and responds to inquiries according to established policies for release of information. Answers questions from nursing personnel and physicians' offices regarding admissions and patients' locations.
    • Responsible for obtaining required signatures for advance directives, financial assistance, and Medicare rules from patients. Refers more complex or difficult questions to supervisor.
    • Prepares medical clinic reports and records information in computer database for processing and billing.
    • Completes forms for following day pre-admissions which includes stamping forms and laboratory slips. Checks status of pre-admissions on scheduled day to determine if patients made appointments or require rescheduling.
    • Directs or escorts patients to assigned units according to established procedures except ICU and Transition.
    • Corrects or updates computer records to reflect changes in patient status, patient transfers, newborn admissions, and so forth. Ensures corrected information is forwarded to appropriate departments.
    • Performs other duties as assigned, some of which may be essential to the job.
    • Requisitions and stores approved office supplies and forms.
    • Files records or documents for retrieval by other departments such as pathology, quality assurance, medical records, or administration.
    • Assists and trains volunteers to prioritize patients, give directions to patients and families, and provides transportation as needed.
    • Is responsible for the day to day department operations, specifically scheduling, coverage needs, and serve as the go to person for issues that arise.


    Education/Training: High School level of communication and/or math skills.

    Experience: 1-2 years of previous experience plus 3-6 months of time on the job.


    • Analytical skills to read and understand instructions, add and subtract numbers, and make relatively straight-forward comparisons between numbers and letters.
    • Typing and/or computer skills.
    • Communication skills necessary to explain policies or procedures, relay patients needs to appropriate personnel, or otherwise communicate with others in situations requiring sensitivity and tact.
    • Work is performed in accordance with instructions and procedures but incumbents must organize the work, set priorities, and request occasional exceptions when necessary.

    Environmental and Job Hazards:

    Physical and Mental Demands: Walking or standing for an hour or more at a time, stooping and bending, reaching and grasping with arms and hands, proofreading and checking document for accuracy.

    Required Skills

    Prior experience in the medical field.
    Proficiency with Word, Excel, and computer skills.
