LICENSED PRACTICAL NURSE (LPN)

OBJECTIVE SUMMARY: Provides professional nursing care for outpatients following established standards and practices. Assesses patient health problems and needs, develop and implement nursing care plans in collaboration with licensed providers and registered nurses, maintains medical records and patient care registries. Serves as a role model, leader, and care coordinator of a multidisciplinary care team. As a professional caregiver the Licensed Practical Nurse is a key member of the Physician Practice and provides clinical and administrative expertise to ensure all patients receive high quality, efficient care.

ESSENTIAL FUNCTIONS:

  • Does not directly manage or supervise, but works in coordination with MAs.
  • Responds to incoming patient telephone calls and voicemails.
  • Makes outbound patient telephone calls.
  • In collaboration with licensed providers telephonically manages chronic care patient needs.
  • Works with providers and patients to reduce hospital re-admission rates.
  • Create action plans with patients to improve health and function.
  • Assists with scheduling of office visits, tests and treatments.
  • Arranges referrals to a specialist when directed by the provider.
  • Observes, records and reports patient’s condition to providers.
  • Maintains and reviews patient records, charts and other pertinent information.
  • Develops, discusses and communicates appropriate problem list (plan of care) for each patient, in collaboration with each provider and the patient/family/significant other in order to address all identified needs.
  • Obtains vital signs and escorts patients to rooms as requested.
  • Performs general nursing care for patients. Administers prescribed medications and treatments ordered by licensed providers.
  • Prepares equipment and aids providers during treatment, examination, and testing of patients.
  • Demonstrates the skills and judgment necessary to implement medical plan of care in collaboration with licensed providers, nursing interventions and procedures as necessary for the care of the patient.
  • Maintains up-to-date and accurate documentation of nursing care provided to facilitate the integration of information for use by the healthcare team to assure quality care.
  • Functions as a role model for current and new staff.
  • Attends all required safety training programs and can describe responsibilities related to general safety, department/service safety and specific job-related hazards.
  • Follows the hospital infection control policies.
  • Demonstrates respect and regard for the dignity of all patients, families, visitors and fellow employees that promotes a professional, responsible and courteous environment.
  • Promotes effective working relations and works effectively as part of a department team inter- and intra-departmentally to facilitate the department’s ability to meet its goals and objectives.
  • Demonstrates dependability/flexibility in meeting scheduling needs of the department.
  • Attends required meetings and participates in committees as requested.
  • Participates in professional development activities and maintains professional affiliations.
  • Maintains patient confidentiality.
  • Instructs patient and family regarding medications and treatment instructions as provided by a licensed provider.
  • Administration of medication via injection, blood draws, preparation of urine specimens, complete lab requisitions and inform physician of abnormal or unusual results.
  • Follows standard precautions using personal protective equipment as required.
  • Participates in quality initiatives and processes as requested.
  • Adheres to the National Patient Safety Goals as outlined by JCAHO.
  • Performs related work as assigned.

REQUIRED EDUCATION: Graduate of an accredited school of Practical Nursing

REQUIRED EXPERIENCE: One year of professional nursing experience in a hospital or clinic setting

PREFERRED EXPERIENCE: Two years of professional nursing experience in a hospital or clinic setting preferred

ADDITIONAL ELIGIBILITY QUALIFICATIONS: Current Washington State LPN License required, BLS required, ACLS preferred

WORK ENVIRONMENT: Works in close proximity to others and/or in a distracting environment. Frequent exposure to bodily fluids, communicable diseases, toxic substances, medicinal preparations and other conditions common to a hospital and/or clinic environment.

PHYSICAL DEMANDS: Requires full body motion, manual and finger dexterity and eye-hand coordination. Requires frequent standing, walking and sitting. Occasionally lifts and carries items weighing up to 50 lbs. Requires corrected vision and hearing to normal range. Requires working under stressful conditions.
